Abstrakt (PL)
Praca poświęcona jest polityce ochrony zdrowia psychicznego, analizowanej w perspektywie globalnej, europejskiej i polskiej, ze szczególnym uwzględnieniem trwającej transformacji polskiego systemu w kierunku modelu środowiskowego. Celem pracy była ocena funkcjonowania polskiego systemu ochrony zdrowia psychicznego oraz identyfikacja głównych wyzwań i pożądanych kierunków jego rozwoju, na tle standardów międzynarodowych, doświadczeń wybranych krajów Unii Europejskiej oraz opinii praktyków zatrudnionych w Centrach Zdrowia Psychicznego (CZP). W części teoretyczno-analitycznej, opartej na analizie danych zastanych, przedstawiono ewolucję podejścia do ochrony zdrowia psychicznego, statystyczny obraz zagrożeń w skali globalnej i europejskiej oraz politykę zdrowia psychicznego Unii Europejskiej. Przeprowadzono analizę porównawczą systemów Danii, Włoch i Niemiec. Wykazała ona, że niezależnie od modelu organizacyjnego systemy te mierzą się z pokrewnymi wyzwaniami: niedokończoną deinstytucjonalizacją, nierównościami terytorialnymi i fragmentaryzacją opieki, a o rezultatach decydują czynniki przekrojowe: stabilne finansowanie opieki środowiskowej, koordynacja międzysektorowa i długofalowe planowanie. Analiza polskiego systemu ujawniła wyraźną lukę między skalą potrzeb a odpowiedzią systemową, której głównymi ograniczeniami są niedofinansowanie, niedobory kadrowe, luki regulacyjne oraz niepewność co do docelowego kształtu reformy. Część empiryczną stanowi badanie własne, zrealizowane metodą sondażu diagnostycznego za pomocą ankiety internetowej wśród praktyków zatrudnionych w CZP. Wyniki ujawniły, że założenia modelu są słuszne, ale wymagają lepszych warunków systemów do skutecznej implementacji. Respondenci poparli dalszy rozwój CZP jako priorytet reformy, wysoko oceniając integrację opieki, punkty zgłoszeniowo-koordynacyjne i lokalną dostępność świadczeń, a zarazem ostro skrytykowali otoczenie systemowe modelu: poziom nakładów na psychiatrię, mechanizmy rozliczeniowe, niestabilność regulacyjną i deficyt koordynacji centralnej.
Abstrakt (EN)
This paper focuses on mental health policy, analysed from a global, European Union and Polish perspective, with particular emphasis on the ongoing transformation of the Polish system towards a community-based model. The aim of the study was to assess the functioning of the Polish mental health care system and to identify the main challenges and desirable directions for its development, against the backdrop of international standards, the experiences of selected European Union countries and the opinions of practitioners employed in Mental Health Centres. In the theoretical and analytical section, based on an analysis of existing data, the evolution of approaches to mental health care, a statistical overview of risks at global and European levels, and the European Union’s mental health policy were presented. A comparative analysis of the systems in Denmark, Italy and Germany was carried out. This analysis showed that, regardless of their organisational model, these systems face similar challenges: incomplete deinstitutionalisation, regional inequalities and the fragmentation of care, whilst outcomes are determined by cross-cutting factors: stable funding for community-based care, cross-sectoral coordination and long-term planning. An analysis of the Polish system revealed a clear gap between the scale of needs and the systemic response, the main constraints of which are underfunding, staff shortages, regulatory gaps and uncertainty regarding the intended shape of the reform. The empirical part consists of the author’s own research, carried out using a diagnostic survey via an online questionnaire among practitioners employed at the centres. The results revealed that the model’s assumptions are sound, but that better systemic conditions are required for effective implementation. Respondents supported the further development of the centres as a reform priority, rating the integration of care, referral and coordination centres, and local access to services highly, whilst at the same time sharply criticising the model’s systemic environment: the level of funding for psychiatry, reimbursement mechanisms, regulatory instability and a lack of central coordination.
