Rekonstrukcja obrazu siebie. Jakościowa analiza doświadczenia kobiet z późną diagnozą ADHD

dc.abstract.enIn recent years, there has been growing interest in the topic of ADHD, particularly in the context of individuals who receive a diagnosis too late, often only in adulthood. Girls and women are particularly vulnerable to receiving a late diagnosis, which is largely due to persistent stereotypes about ADHD and insufficient knowledge among specialists regarding sex differences in the presentation of characteristics of this disorder. As a consequence, many women do not receive a correct diagnosis or obtain one only in adult life, which means that for many years they have to function with difficulties resulting from a disorder of which they are unaware. A lack of awareness of the causes of the problems they experience may have serious consequences for many areas of an individual's functioning and may negatively affect psychological well-being. The absence of early recognition makes it more difficult to identify the sources of symptoms and to understand one's own behaviours. For this reason, women's self-image may be shaped on the basis of inaccurate and unrealistic beliefs about themselves. The aim of the study presented in this thesis was to understand the significance of a late ADHD diagnosis (after the age of 18) for women's self-image, as well as to identify its role in the process of reconstructing and reinterpreting their own experiences and behaviours. A qualitative approach was applied, using the method of Interpretative Phenomenological Analysis (IPA). The study took the form of in-depth, semi-structured interviews. Eight women aged between 20 and 39 took part in the study, all of whom had received a formal ADHD diagnosis after the age of 18. Three main temporal perspectives emerged from the interviews, serving as points of reference for the participants' experiences: life before diagnosis, diagnosis as a turning point, and the period after diagnosis. Themes related to each of these periods were also identified. The results indicate that women's self-image before receiving an ADHD diagnosis was based on beliefs about their own incompetence, inadequacy, and laziness. The diagnosis constituted an important turning point in the women's lives, enabling a change in the meaning attributed to past experiences, a transformation of their vision of the future, and a gradual reconstruction of their previous self-image.
dc.abstract.plW ostatnich latach obserwujemy wzrost zainteresowania tematyką ADHD, szczególnie w kontekście osób, które otrzymują diagnozę zbyt późno, często dopiero w dorosłości. Szczególnie narażone na otrzymanie późnej diagnozy są dziewczynki i kobiety, co w dużej mierze wynika z utrzymujących się stereotypów dotyczących ADHD i niewystarczającej wiedzy specjalistów na temat różnic międzypłciowych w prezentacji cech tego zaburzenia. W konsekwencji, wiele kobiet nie otrzymuje prawidłowej diagnozy lub uzyskuje ją dopiero w dorosłym życiu, co sprawia, że przez wiele lat muszą one funkcjonować z trudnościami wynikającymi z zaburzenia, którego nie są świadome. Brak świadomości przyczyn doświadczanych problemów może mieć poważne konsekwencje dla wielu obszarów funkcjonowania jednostki i negatywnie przekłada się na dobrostan psychiczny. Brak wczesnego rozpoznania utrudnia poznanie źródeł objawów i zrozumienie własnych zachowań. Z tego powodu obraz siebie kobiet może być kształtowany w oparciu o niewłaściwe i nierealistyczne przekonania na temat siebie. Celem prezentowanego w tej pracy badania było zrozumienie znaczenia późnej diagnozy ADHD (po 18 r.ż.) dla obrazu siebie kobiet, a także zidentyfikowanie jej roli w procesie rekonstrukcji i reinterpretacji własnych doświadczeń oraz zachowań. Zastosowano podejście jakościowe wykorzystujące metodę Interpretacyjnej Analizy Fenomenologicznej (IPA). Badanie miało formę pogłębionych, półustrukturalizowanych wywiadów. Uczestniczyło w nich osiem kobiet w wieku od 20 do 39 lat, które otrzymały formalną diagnozę ADHD po ukończeniu 18. roku życia. Z rozmów wyłoniono trzy główne perspektywy czasowe, stanowiące punkty odniesienia do doświadczeń badanych (życie przed diagnozą, diagnozę jako punkt zwrotny oraz czas po diagnozie) oraz wyłoniono tematy związane z każdym z tych okresów. Wyniki wskazują, że obraz siebie kobiet przed diagnozą ADHD opierał się na przekonaniach o własnej niekompetencji, niewystarczalności i lenistwie. Diagnoza stanowiła istotny punkt zwrotny w życiu kobiet, umożliwiając zmianę znaczenia przypisywanego przeszłym doświadczeniom, przekształcenie wizji swojej przyszłości oraz stopniową rekonstrukcję dotychczasowego obrazu siebie.
